Patient Education on Access

Hi all,
I am a nursing student on an internship at a hospital. I have been given an assignment to find (not make) a low-literacy patient education sheet for patients who will need a fistula in the future. The purpose of the material must be to teach the patient how to protect the future site, such not letting healthcare workers take blood pressures or draw blood on the arm that will be used for the access. I have been searching high and low, does anyone know if such a thing exists? Thank you so much.

The Centers for Medicare and Medicaid Services has started an initiative through the End Stage Renal Disease Networks called "Fistula First" to educate all the "players" about the benefits of fistulas. Here's the link to the patient education materials. See especially the patient resource list:
