Patient Scrapbook
The Life Options Patient Scrapbook exists to share photos of patients exercising, playing with grandkids, working—to show that life with kidney failure can be full and active!
We are looking for color photos from all varieties of dialysis patients—younger, older, male, female, every ethnic group, people who are walkers, bike riders, and those more limited in their abilities. So, please join our Life Options Patient Scrapbook. You could be a star and inspire other patients!

Include your name, age (optional), type of treatment, how long you've had kidney failure, and a line that says, "I give Life Options permission to use my photo and information about me I have provided for its website," and sign your name, date, and telephone number. If you prefer, we are happy to use only your first name to identify you.
Digital photos can be e-mailed in either .jpg or .tif format. Include in your e-mail permission for use.
